Scope of practice, competencies and impact of advanced practice nurses within APN-led models of care for young and middle-aged adult patients with multimorbidity and/or complex chronic conditions in hospital settings : a scoping review

This scoping review examines the role of advanced practice nurses (APNs) in the care of younger and middle-aged people with multiple chronic conditions in hospital. The results show that APNs, with their specialised expertise, take on important tasks in transition management, direct patient care and team collaboration. Although the models are designed very differently, all studies show positive effects on care. Dr. Hanna Mayer from the Division of Nursing Science with a focus on Person-Centred Care Research was significantly involved in this article.
